File a Complaint with the Civilian Review Board
Once the complaint form has been completed, a signature is required in order for it to be eligible for review by the CRB. If submitting online, please send the completed form to us here by clicking here and a staff member will contact you to set up the electronic signature process.
If you are unable to sign electronically, a staff member will mail you the form for signature with a stamped, self-addressed envelope for return.
