Once the complaint form has been completed, a signature is required in order for it to be eligible for review by the CRB. Once you submit your completed complaint form, a staff member will contact you and set up an electronic signature process. If you are unable to sign electronically, a staff member will mail you the form for signature with a self addressed and stamped envelope. 

You can begin the process of filing a complaint with the Civilian Review Board by completing the complaint notification form below.

Once completed, the notification will be forwarded to Board staff for assessment. Staff will contact you within two (2) business days to instruct you on how to complete the filing process. Please note: Your complaint must be filed on a form authorized by the Board in order for the complaint to be eligible for the Board’s consideration.
